Right to Choose Referral for ASD & ADHD Assessments
We understand that seeking an assessment for Autism Spectrum Disorder (ASD) or Attention Deficit Hyperactivity Disorder (ADHD) is an important step in understanding your health and accessing appropriate support.
Through the NHS Right to Choose initiative, you have the option to choose your preferred provider for these assessments.
Steps to Initiate a Right to Choose Referral

Discuss with Your School and SENCO
If you are a student, it's essential to first discuss your concerns with your school and the Special Educational Needs Coordinator (SENCO). They can provide valuable insights and support in the referral process. Please ask them for a report highlighting the difficulties which can be useful in preparing the referral.
- Choose a Right to choose provider and contact them to ensure they are able to help with assessment and treatment if this is what you require.

Complete a Screening Questionnaire
To assist in the referral process, please complete the following self-assessment questionnaires:

AQ10 (Autism Spectrum Quotient - 10): A brief screening tool to help identify traits associated with ASD.

Wender Utah Rating Scale (WURS): A retrospective assessment to identify ADHD symptoms experienced during childhood.
These questionnaires are designed to provide an initial indication of ASD or ADHD traits and are not diagnostic tools.

Provide Supporting Information
Along with the completed questionnaire, please submit information that outlines how the condition affects your daily life. This may include:

Challenges in social interactions or communication

Difficulties with attention, focus, or impulse control

Impact on academic performance or workplace productivity

Emotional or behavioural concerns
This information will help us understand your situation better and support your referral effectively.

Submit Your Referral Request
Once you have gathered the necessary information, please contact our team to submit your referral request. You can do this by bringing the information to the surgery or sending the information through.
Our team will review your information and assist you in initiating the Right to Choose referral process.
