Firearms Licence Forms

These are specific forms and can be for a new application for a firearms licence or a renewal of your firearms licence.

Please submit your forms to the surgery signed by yourself or we can provide the completed form which will require your signature on collection.

There is a fee of £120 for a new application and £80 for a renewal and please allow up to 28 days for the GP to do this non-NHS work.
