Travel Cancellation

Gps can only provide travel cancellation documents if we have evidence of health conditions which affected your ability to travel.

Please check with your insurance company if they need a letter from your GP or a completed form.

We often find that they will need a specific form to be completed so please check with your insurer first before requesting a letter from your doctor.

The form will need to be submitted to the surgery so your GP can complete it, at this time we area unable to accept email letters so ask that you bring it to the surgery.

Please note that there is a fee of £40 for doing this non-NHS work for you.

Please allow up to 28 days for the GP to do this non-NHS work.
